Here are the forms needed for the 2008-2009 Henry Clay Band students.  Please fill these out and bring them to the first day of band camp for your child.  Please note that the Medical Release Form is required to be completed and turned in before participation in band camp!  Also note that none of the forms are required to be notarized.
